Before your appointment:

• Complete the Healthie forms online.  You may use the link above or you should have received an email with login information.  

• Request your medical records and growth charts (if a pediatric patient) to be sent
  to LifeCycle Nutrition via fax or mail

 

What to Bring to your appointment:​

• Driver's license

• Any medical records that were not sent directly to us
